Welcome to the practice. To register with Court Thorn Surgery, please bring your medical card to reception. If you are unable to find your medical card the receptionist will assist you and give you appropriate advice. You may also download the following documents to register.

We encourage all patients over the age of five to have a new patient check. The receptionist will provide you with a form to fill out, giving details of your medical history or you may download the attached document and bring it to the surgery.

Practice Charter
We aim to:
* Provide a personal friendly and confidential service of a high standard.
* Treat all patients with courtesy and respect whatever their culture, beliefs, sexual orientation.
* Keep patients informed about our services and provide other healthcare information.
* Endeavour to see you within 10 minutes of your appointment time and inform you of any delay.
You agree to:
* Treat us courteously and respect the surgery and its surroundings.
* Ask for help outside surgery hours, only when your problem cannot wait until the surgery reopens.
* Use the appointment system, except in case of a real emergency or difficulty, and if you have to cancel, please do so early so that another patient can take your place.
* Request home visits only when you are too ill to come to the surgery.
* Understand that emergencies do happen from time to time and may disrupt clinics.
You have the right to:
* Be offered a health check when you join our practice for the first time.
* Be offered a chaperone, either your representative or trained member of staff.
* To receive information about the services your GP provides and to see on request a copy of the practice leaflet which sets out this information.
